I have a high uric acid level so any knee or foot pain is immediately diagnosed as gout. But the recognised palliative (colchichine) only gives me diarrhoea (the recognised side effect after 36 hours) and other painkillers (ibuprofen, paracetamol, aspirin, etc.) are ineffective and only risk aggravating an existing high blood pressure condition. The long term recommendation is Allopurinol to reduce the uric acid level. There is also a view that I have Ankylosing Spondylitis (blood indicators, fused sacroiliac joints, iritis, etc) so the pain is sometimes attributed to that. I have found that sometimes steroids do reduce the symptoms dramatically over a couple of days. None of the medics I've seen are happy with this as a palliative measure, but it makes life possible again! -- Graham J

I'm lucky; I take Allopurinol every day, it doesn't make me ill, and it dramatically reduces the times that I get gout. I tried that- went to seee s specialist and got a list of foods to avoid. Top of the list was shellfish. Well sod that, I love shell fish too much. And anyway, following all the edvice seemed to make little real difference. So I've settled for a (remaining) life time of allopurinol. -- Peter

It blocks the last enzyme in the pathway that converts purines into uric acid - the molecule that crystallises out from the blood at high concentration as shards of "glass" into your joint spaces. I have been on it for years and gout free for all of that time. The next best thing for me were the NSAID's (anti-inflammatory) - Diclofenac being the best for me. Definitely reduced swelling and inflammation and reduced the pain - I topped 'em up with Ibuprofen too (which is also anti-inflammatory). They will eat through your stomach wall though if you don't take them with food. Whiskey helps too ! -- JohnB
