Maybe they were OK, and then you subsequently did something that changed
them to something bogus?

The top icon's the physical disk, and then inside that (ie indented) are all
the partitions on the disk.

I think you need to select the partition and then repair permissions.

(Though personally I just do "sudo diskutil repairPermissions /" on the
command-line. Since / indicates a filesystem, that also suggests that you
need to select the partition icon.)
