Actually we have decided to drop the convio+sdixon was configured as a
secondary email address, and perhaps recreate it without the + sign and see
if that makes a difference. Thanks for you help so far.
However while we are at it, based on your experience do secondary smtp
addresses create problems with OWA access, if they are created without the
special characters?

The + sign is used by IE as an escape character (replacing a space), so IIS
interprets "convio+sdixon" as "convio sdixon". If you are saying that the
user's email address actually has a plus sign in, then this might be the
cause of the confusion, since OWA pre-2007 uses smtp addresses to form part
of the user's OWA URL. As far as I can tell, it uses the oldest email
address that the user has in the primary SMTP domain, so if your user has an
email address, say sdixon(a)whatever.com, then delete all the others, and
re-add them (if you need to), thus making sdixon the oldest one. This
should become their new OWA URL that they get automatically redirected to
after logging in, and since it has no escaped characters, the problem will
hopfully go away.
